1 Assistant team leader part time – line management/oversight of processes/oversight of transitions tracker (plus other duties not related to transitions); 1 Group Manager – ad hoc management support/budget forecasting/strategic commissioning (plus other duties not related to transitions); 1 General Manager - budget forecasting/strategic commissioning (plus other duties not related to transitions).
The following health professionals are available to provide support as and when required depending on the specific health needs of the individuals.
2 x Psychiatrists; 2x Psychologists; 2 x Music Therapists; 2 Speech and Language Therapists; 1 Physiotherapist; 1 Occupational Therapist; 1 Dietician; 6 staff nurses; 1 charge nurse; 2 senior charge nurses; 1 advanced nurse practitioner
* Local Area Co-ordination/Community Link Workers may also provide support with transitions. Local Area Co-ordinators/Community Link Workers working learning disability (5.61fte) offer support to some young people in transition. LAC Team Manager – input into LD transitions tracking process and strategic oversight of LD LAC work in transitions. 1.6fte LAC Service Managers – line management and operational oversight of LD LAC staff (plus full operational responsibility for adult mental health LAC, older adults and adults with a physical disability LAC)

Across children and adult services there are staff who may support transitions work at any given time as part of wider caseloads for all ages. These include social workers, paraprofessionals, community link workers and management support.
Children Affected by Disability (CHAD) social workers are allocated to work with children until the age of 18. They are therefore part of the transition planning for young people making a transition into adult services working alongside adult social work colleagues. In the CHAD team there are currently 4 social workers who cover the Scottish Borders (separated into Geographical areas). They are managed by 1 team leader.
In education all secondary schools have responsibilities under the Additional Support for Learning Act legislation to plan for post-school transitions and to share relevant information with service providers who will be supporting young people post-school six months in advance of the transition. These responsibilities are fulfilled by DHT’s and Support for Learning PTS in each of our nine secondary schools.
There are specific additional responsibilities the five PT’s of the 5 enhanced provisions to liaise on behalf of young people leaving school with, as appropriate in each case, partners including SDS, DYW, social work, Borders College and employment support services to plan for this. In additional schools provide regular updates (currently via the Educational Psychology Service) to the Learning Disability trackers group and the Principal Educational psychologise represents the education service on this group.
The 16+ co-ordinator is involved in the tracking and planning of this group of young people.
We are an all age Employability Service so we have Employability Workers working with all ages not specific to only young people and those with Disability, however like many services we support transitions in various forms, ours being employability related support such as additional support through key workers to help people find and sustain paid employment or those requiring employability training.
