Use this form to request PPE if you need to provide care to family, friends or neighbours, or for the person providing care to you.

PLEASE NOTE – PPE requests made via this online form will only be actioned Monday – Friday. If you require emergency supplies of PPE out with these times please call Customer Advice and Support on 0300 100 1800. You may be directed to an alternative ‘out of hours’ emergency contact number when our call centre is closed, you will be advised of this if necessary.
